myEndodontist is committed to providing you with the highest standard of dental care. In order to achieve this, we are required to collect Personal Information about you. This privacy policy provides information about the way we collect and manage your personal information.

How and what personal information do we collect?

myEndodontist may collect your personal information when you contact us via telephone calls, email, SMS, social media, and our website. We may collect your information directly from you, or any person with your authority for us to seek information from, such as a family member, a carer, other healthcare professionals, your health fund, Medicare, or the Department of Veterans’ Affairs. We also collect information from referring dentists so that we can report back to the referring dentist following treatment. This information includes basic details such as your name, date of birth, contact details and health insurance information. It is also necessary for the endodontist to obtain from you details regarding your general health and past medical or surgical events. Without this general health history, the endodontist is unable to plan your care adequately and may place you at undue medical risk.

Naturally, some of this information is of a personal nature and some of it might be regarded as ‘sensitive’ and not the sort of information that you would wish to be unnecessarily disclosed to others.

If you provide your personal information to us you consent to us collecting, holding, using, and disclosing that information in accordance with this privacy policy.

Dealing with us anonymously

You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals.

Who do we disclose personal information to?

If you are a patient, we may disclose your personal information to other healthcare practitioners involved in your care and your health insurer, or DVA.

We will not otherwise disclose your personal information to third parties unless we are permitted or required to do so by law or with your express consent.

Our practice will not use your personal information for marketing any of our services directly to you without your express consent.

How can you access or seek correction of your personal information we hold about you?

If you wish to request access or correction of your personal information, please submit your request in writing via info@myendodontist.com.au.

In order for us to respond to your request, you will need to provide us with your full name, address, and a description of the information you wish to access, or specify the manner in which the information we hold is inaccurate, out of date, irrelevant, incomplete or misleading and how it should be amended.

How will we deal with any complaints in relation to how we handle your personal information?

Please contact us through the practice email at info@myendodontist.com.au if you are not happy with how we handled your personal information and you are concerned that we have breached an obligation under the Australian Privacy Principles or this privacy policy.

We will consider your complaint and determine whether it requires further investigation. We will notify you of the outcome of this determination and/or any subsequent internal investigation.
